When the issue is whether a patient has ischaemia, the finding of typical strain features of ischaemia in more than just a single segment favours ischaemic dysfunction rather than noise artefacts. This is illustrated in Figure 5 which shows mild reduction in systolic shortening and post-systolic shortening in several adjacent segments. An interesting application of GLS is in the evaluation of patients with suspected stable angina pectoris where it was shown to be an independent predictor of significant coronary heart disease, at rest and during dobutamine stress echocardiography.16,17 Another promising application of strain imaging is identification of the relatively large subgroup of non ST-elevation myocardial infarction patients with total coronary occlusion, who needs urgent revascularization.18 Lack of ST elevation in these patients reflects limited sensitivity of electrocardiogram (ECG) in identifying patients with coronary occlusion.19 Post-systolic strain has been proposed as a marker of viability, but should not be used as a stand-alone index since post-systolic shortening also occurs also in myocardium with transmural necrosis or scar (Figure 6). In the latter case, there is typically systolic lengthening, and post-systolic shortening is due to passive recoil when LV pressure is falling during isovolumic relaxation. Furthermore, load-dependent interactions with non-ischaemic myocardium can modify systolic strain.20,21 When there is systolic hypokinesia or akinesia indicating some degree of active contraction, post-systolic shortening is attributed to active contraction and reflects viable myocardium.14 Importantly, a segment which is entirely passive during the first few hours after coronary occlusion, may yet not be irreversibly injured and may recover with reperfusion.15 In the chronic phase after an infarct, however, an entirely passive strain curve is most likely a sign of scarring. There are limited clinical data to verify these concepts which have been well documented in experimental models. A further study showed no statistical difference between the use of 20 or 40 units of hyaluronidase in degrading 0.2mL (4 to 6mg of HA) of various fillers.23 Nodules, and product that has been injected into the superficial dermis should be injected directly, injections should be placed immediately into and below the product.38[[Not sure what the previous sentence is trying to convey]] For vascular compromise, serial puncture should be used to inject hyaluronidase along the course of the vessel and cover the affected area.4 The needle should be perpendicular to the skin and several injections are often necessary. Hyalase (hyaluronidase 1500 units) has an off-license use in aesthetic medicine and except in the case of emergency administration requires the patient to undergo a skin patch test at least twenty minutes prior to the procedure being undertaken. The skin patch test is carried out by injecting Hyalase into the subcutaneous tissue of the forearm and observed for signs of reaction (i.e. hives or wheals). If a positive patch test result is observed, treatment with Hyalase cannot be carried out. Erythema or redness and slight vasodilation may be expected. A hyaluronic acid (HA) filling treatment performed by an inexperienced specialist can often end up with complications such as asymmetry or lump formation. The acid may also move in the skin after the procedure. Fortunately, it is possible to dissolve unnecessary hyaluronic acid using hyaluronidase. Objective: To systematically map the available evidence and identify the gaps in knowledge on the effectiveness of hyaluronidase use in managing the aesthetic complications associated with HA injections (vascular occlusion, blindness, nodules, delayed hypersensivity, granuloma, poor aesthetic outcome). Methods: PubMed, Medline, Embase and Cochrane databases were used up to May 2022, to look for randomized clinical trials (RCTs), clinical trials, and retrospective case-control studies reporting on the use of hyaluronidase for managing the HA filler injection complications. Results: The database search yielded 395 studies; of those 5 RCTs (all carried out in the USA) were selected (53 subjects), indicating the effectiveness of hyaluronidase for removal of un-complicated injected HA nodules (forearm, upper arm, or back skin). The follow-ups ranged from 14 days to 4 years. The amount of HA filler injected into each site varied from 0.2 to 0.4 mL. A dose dependent response was observed for most HA fillers. No major adverse reactions were reported. Overall, for removal of every 0.1 mL of HA filler they injected 1.25-37.5 units of hyaluronidase (single injections). When 3 consecutive weekly hyaluronidase injection was used much lower doses of 0.375-2.25 unit was utilised. There was no evidence in a form of RCTs, clinical trials, and retrospective case-control studies on the removal/reversal of HA injections in the facial skin, or management of over-corrections, inflammatory nodules, or tissue ischemia/necrosis associated with HA filler injection. Conclusion: Based on studies on the forearm, upper arm and back skin, hyaluronidase can be used for the reversal of uncomplicated HA filler injection nodule. However, further adequately powered studies are warranted to establish the ideal treatment protocol/dose of hyaluronidase for reversal of HA filler injections in the facial region or management of complications associated with aesthetic HA injection. Level of evidence iii: This journal requires that authors assign a level of evidence to each article. The cumulus-corona-oocyte complex, composed of cumulus granulosa cells embedded in a matrix of hyaluronan oligosaccharide chains cross-linked by hyaluronan binding proteins and proteoglycans, surrounds each oocyte and must be removed prior to intracytoplasmic sperm injection (ICSI). This is traditionally achieved using enzymatic digestion of the matrix with a bovine-derived hyaluronidase followed by mechanical denudation through pipetting. A human recombinant hyaluronidase (Cumulase) has been developed with the intent of circumventing the problems and concerns associated with the animal origin and lack of purity of the bovine-derived form of the enzyme. In order to compare the effect of Cumulase with that of the bovine enzyme on the rates of normal fertilization and oocyte damage, a retrospective study using four experienced practitioners was performed. In 2006, using Cumulase, a significantly increased rate of normal fertilization (P = 0.0003) and a significantly decreased rate of oocyte damage (P < 0.0001) were observed compared with 2005, during which time bovine-derived hyaluronidase was predominantly used. This study indicates that Cumulase is safe and effective for use in the removal of the cumulus-corona-oocyte complex prior to ICSI, and may have several distinct advantages over the animal-derived form of the enzyme in terms of safety and efficacy. As defined in Aetna commercial policies, health care services are not medically necessary when they are more costly than alternative services that are at least as likely to produce equivalent therapeutic or diagnostic results. Herceptin (trastuzumab), Herceptin Hylecta (trastuzumab and hyaluronidase-oysk), Herzuma (trastuzumab-pkrb), Ontruzant (trastuzumab-dttb), and Trazimera (trastuzumab-qyyp) are more costly to Aetna than other Anti-HER2 Monoclonal Antibodies. There is a lack of reliable evidence that Herceptin (trastuzumab), Herceptin Hylecta (trastuzumab and hyaluronidase-oysk), Herzuma (trastuzumab-pkrb), Ontruzant (trastuzumab-dttb), and Trazimera (trastuzumab-qyyp) are superior to the lower cost Anti-HER2 Monoclonal Antibodies: Kanjinti (trastuzumab-anns) and Ogivri (trastuzumab-dkst). Therefore, Aetna considers Herceptin (trastuzumab), Herceptin Hylecta (trastuzumab and hyaluronidase-oysk), Herzuma (trastuzumab-pkrb), Ontruzant (trastuzumab-dttb), and Trazimera (trastuzumab-qyyp) to be medically necessary only for members who have a contraindication, intolerance, or ineffective response to the available equivalent alternative Anti-HER2 Monoclonal Antibodies: Kanjinti (trastuzumab-anns) and Ogivri (trastuzumab-dkst). Aetna considers continuation of trastuzumab and hyaluronidase-oysk (Herceptin Hylecta) therapy medically necessary in members requesting reauthorization for an indication outlined in Section I when there is no evidence of unacceptable toxicity or disease progression while on the current regimen. Welcome to the 184th Carnival of Mathematics with your host Dr Tom Crawford. After accidentally starting a trend (see Carnival 167) with listing fun facts about the number of the carnival, here are my favourites about the number 184: Mark Bollman is Professor of Mathematics and chair of the Department of Mathematics & Computer Science at Albion College in Albion, Michigan, and has taught 113 different courses in his career. Among these courses is \"Mathematics of the Gaming Industry,\" where mathematics majors carefully study the math behind games of chance and travel to Las Vegas, Nevada, in order to compare theory and practice. He has also taken those ideas into Albion's Honors Program in \"Great Issues in Humanities: Perspectives on Gambling,\" which considers gambling from literary, philosophical, and historical points of view as well as mathematically. Mark has also authored Basic Gambling Mathematics: The Numbers behind The Neon and Mathematics of Keno and Lotteries. It is in the interest of the carnival hosts to sell as many tickets as possible. One way to encourage this is to make the larger groups of tickets less expensive per ticket. The way they have set it up, the groups of 25 tickets offer the best value. No one should purchase groups of 50 tickets because it is a better deal to buy two groups of 25 tickets and similarly no one should purchase groups of 120 tickets because it is a better deal to purchase 5 groups of 25 tickets. Since launching in 2014, the annual carnival has traveled to various parts of Mississippi to give students the opportunity to learn more about science, technology, engineering and math principles in a fun, interactive way. This year, MSMS students and faculty hosted the Traveling STEM Carnival for a multitude of fourth grade classes from 33 Jackson Public Schools. During the carnival, students engaged in eight different exhibits that included hands-on activities, science experiments and math-based competitions. The writer of a book on mathematics for laymen, no matter how hard he tries to avoid technical jargon and to relate his subject-matter to reader interests, faces a similar problem: How can he keep his readers turning the pages The best way, it has always seemed to me, to make mathematics interesting to students and laymen is to approach it in a spirit of play. On upper levels, especially when mathematics is applied to practical problems, it can and should be deadly serious. But on lower levels, no student is motivated to learn advanced group theory, for example, by telling him that he will find it beautiful and stimulating, or even useful, if he becomes a particle physicist.
